Tadalafil (also known as ‘generic Cialis’) is a tablet for erectile dysfunction. It is the non-branded version of a well-known drug used in the treatment of male impotence. The branded treatment is manufactured and marketed by Lilly, however the patent on this medicine expired in 2017. Erectile dysfunction is a condition which is perhaps more common than many suspect. It is thought that around half of men over the age of 40 will encounter the condition at some point, although men of any sexually-active age can experience ED. Also known as impotence, it is characterised by an inability to achieve an erection during sexual intercourse, or maintain one long enough to have penetrative sex.
There are a number of reasons why men experience the condition. Sometimes these might be psychological in nature: often men who are in a new relationship will, for instance, be eager to impress their new partner. This can lead to a high pressure situation, where nervousness can impede erectile function. This is commonly referred to as ‘performance anxiety’. At other times, a man might be distracted by other issues in their life, such as a burgeoning workload in their professional life. This might also affect their performance during sex, and induce erectile problems.
It’s also very common for erectile dysfunction to be self-perpetuating. The worry of not being able to get an erection might make it harder to achieve one, creating something of a cycle.
In other cases, erectile problems may come about as a result of physical issues. These are often rooted in poor blood flow, which is why ED is so often (albeit not exclusively) a problem for older men. As men approach middle age, the risk of high blood pressure and other conditions inhibiting circulation increases, which can lead to erectile dysfunction.
Lifestyle factors can also contribute to impotence. Men who smoke, drink alcohol to excess, keep a poor diet or don’t exercise enough may typically be more susceptible to developing erectile dysfunction due to physical factors, than those men who maintain a healthy routine (by not smoking, limiting alcohol consumption, eating healthily and exercising regularly).
Many men are able to tackle the condition by making positive improvements to their lifestyle. Men who have psychologically-induced ED may also benefit from speaking to their partner about the issue, or by directly addressing the problem responsible (such as a professional workload) for causing them anxiety and stress.
For men who have ED due to physical reasons, treatment can help if their symptoms have not responded to changes in lifestyle.

How does Tadalafil work?
Tadalafil is a PDE5 inhibitor, which works by increasing blood flow to the penis.
PDE5 is an enzyme which is naturally present in the body and helps to regulate the flow of blood. In men with ED, this enzyme has a restrictive effect on blood vessels, which means that blood is not as able to enter the shaft in the penis (corpus cavernosum), a process necessary for an erection to occur.
Tadalafil reduces the effects of this enzyme, which allows blood to travel to and enter the penis more ably. This allows the user to achieve and sustain an erection suitable for penetrative sex.
Please note that Tadalafil is not an aphrodisiac. You will need to be sexually stimulated in order for it to work.
Tadalafil Daily and Tadalafil ‘Weekend’
The drug will be produced in two forms. The daily form is taken every day, and is essentially ‘effective’ all of the time. The weekend form is taken on demand, and the effects last for up to 36 hours. (The term ‘effective’ does not mean a constant erection. This means that the drug will help to produce an erection when sexually stimulated during the ‘effective’ time period. If you develop an erection which does not go away or lasts for more than four hours continuously, seek medical attention right away.)

The below applies to all doses of Tadalafil
- Tadalafil increases blood flow to the penis. It will therefore not provide sexual stimulation and work on its own. You will still need to be sexually stimulated for it to work.
- The effectiveness of Tadalafil may be lessened if taken following a large meal or while drinking alcohol. Grapefruit juice can also cause interactions, and should be avoided if you are using Tadalafil.
- It is advised to limit your alcohol consumption if you have erectile dysfunction. Drinking alcohol can, for a short time, lower blood pressure, and increase the risk of ‘postural hypotension’ (dizziness when standing up) in men taking Tadalafil.
- Never exceed the dose recommended by your prescriber.
- If you develop side effects, or if the dose you are taking is not effective, speak to your doctor.
- During consultation, your doctor needs to be told about any pre-existing medical conditions or other treatments you are using.
- Never take recreational drugs if you are also taking erectile dysfunction medication.

Not everyone who uses Tadalafil will develop side effects, but it is important that you are aware of the side effects associated with it. See a doctor immediately if you notice any signs of:
- an allergic reaction (hives, swelling of the face, lips or throat, or difficulty breathing)
- chest pain
- priapism (an erection lasting for more than four hours and does not go away)
- sudden sight or hearing loss
- or any other side effects that become serious.
Common (1 in 10 people or less):
Headache, back and muscle pain, flushing and redness, stuffy nose and indigestion.
Uncommon (1 in 100 people or less):
Feeling dizzy, stomach discomfort, nausea and vomiting, reflux, blurred vision, eye pain, breathing difficulty, blood in the urine, prolonged erections, palpitations, change in heart rate, hypertension, hypotension, tinnitus, swelling of the hands, feet or ankles, tiredness.
Rare (1 in 1000 people):
Fainting, seizures and temporary memory loss, swelling eyelids, redness in the eyes, sudden hearing loss, hives, penile bleeding or blood in the semen, and increased perspiration, heart attack or stroke.

Taking it with other medications
Tell your prescriber about any other treatments you are using when undergoing consultation for Tadalafil.
You must not take Tadalafil if you are on nitrate medication.
During consultation, it is particularly important to make your doctor aware if you are taking any of the following, as they may interact with Tadalafil: alpha blockers or any other high blood pressure treatment; riociguat; medicines for benign prostatic hyperplasia called ‘5-alpha reductase inhibitors’; antifungal medicines such as ketoconazole; protease inhibitors for HIV or AIDS; phenobarbital, phenytoin or carbamazepine (anticonvulsants); rifampicin, erythromycin, clarithromycin or itraconazole (used in infections); or other impotence treatments.
Conditions to look out for
Tell your doctor about any medical problems you have or have had in the past during consultation.
You must not take Tadalafil if you: are allergic to any ingredients in it; have had a heart attack in the previous 90 days, or have serious heart disease; have had a stroke in the last six months; have low blood pressure or high blood pressure which is not being controlled; have ever had sight loss due to ‘stroke of the eye’ (non-arteritic anterior ischemic optic neuropathy, or NAION).
It is very important to let your doctor know if you have: a heart condition; sickle cell anaemia; multiple myeloma; leukemia; any deformity of the penis; an erection lasting for longer than four hours; or a serious liver or kidney problem.
Alcohol and grapefruit juice
Men who have erectile dysfunction should note that excessive alcohol consumption can worsen symptoms. Drinking alcohol and taking Tadalafil at the same time can increase the risk of dizziness when standing up.
Grapefruit juice can also interfere with the function of Tadalafil. It is better to avoid drinking grapefruit juice when using this treatment.
You must not take recreational drugs if you are taking Tadalafil. Doing so can cause be very dangerous.
Pregnancy and breastfeeding
This drug should not be used by women.
The leaflet advises that while a reduction in sperm was noted in some men in testing, this is not likely to lead to infertility.
Driving and using machinery
If you feel dizzy or tired after taking this medication, do not drive or use machines.
Tadalafil pills contain lactose. Tell your prescriber if you are lactose intolerant.
Can I still drink alcohol?
Drinking alcohol can exacerbate ED, and doing so while using Tadalafil tablets may increase the risk of ‘postural hypotension’. You should limit your alcohol consumption if you are using this treatment.
Will I still be able to drive?
Yes, provided you experience no side effects which affect your ability to do so. If you do, avoid driving and tell your doctor as soon as possible.
How do I store it?
In a safe place, out of the reach of children and at room temperature.
Am I allergic to anything in the medicine?
This drug contains: tadalafil, lactose monohydrate, croscarmellose, hydroxypropyl cellulose, sodium laurilsulphate, microcrystalline cellulose, magnesium stearate, opadry yellow, hypromellose, titanium dioxide (E171), triacetin, talc, iron oxide yellow (E172), or iron oxide red (E172).
Do not use it if you allergic to any of these ingredients.
